InDiablo 4,rarity is relative. Early on in your adventure, the sight of a legendary item dropping will make your day, and that item will likely go on to occupy one of your gear slots for the foreseeable future. Later on though, you’ll see legendary items after every few kills, their once-sparkling edge dulled by familiarity.

No matter how deep into the game you go, however, there are some items that will always be rare: some uniques that you’ll never be able to find reliably, if at all. These items are so rare, in fact, that their existence had to be officially confirmed by the developers on social media. These are the six rarest uniques in Diablo 4, and woe betide all ye who seek them.
6The Grandfather
Appropriately enough, the Grandfather has a long-running legacy within the Diablo series. Beginning in the first Diablo, where it was considered one of the most powerful unique swords in the game, the Grandfather went on to appear in Diablo 2 and Diablo 3 also, where it reprised its role with aplomb. In Diablo 4, its power is similarly monstrous.
Not only does it pack a huge base damage value, but it also increases your raw damage, maximum life, critical strike damage, and general stats by incredible amounts. The Grandfather also includes a sneaky line of text that allows all of its other properties to exceed their normal limits; a kind of built-in limit break, if you will. Though you’ll need to reach the ripe old age of level 85 to even have a chance of glimpsing it, The Grandfather will make one hell of a retirement present if you do.
5Ring Of Starless Skies
If you run a build designed around a core skill, which you most likely do, then the Ring of Starless Skies may be just the trinket for you. Not only does it increase your core skill damage by up to 19.5 percent, but it also reduces the resource cost of each consecutive core skill you use, up to a maximum of 40 percent.

This lets you spam your key moves with impunity, and overwhelm your enemies by all but ignoring the resource systems that were intended to keep the game balanced. On top of that, it provides solid resistance to both cold and fire damage, and raises your odds of landing both critical and lucky hits, as well as the damage that the former deals. It may not have quite the power of the One Ring, but in Diablo 4 it’s the closest thing you’re going to get. Just be sure not to let your hunt for it turn into an all-consuming obsession.
4Andariel’s Visage
As the face of one of Diablo’s storied lesser evils, Andariel, the Maiden of Anguish, Andariel’s Visage has a lot to live up to right out of the box. Thankfully, it more than does so. As a unique helm, it offers solid defensive boosts, as well as an increase in attack speed and an added lifesteal element. This lets you hit enemies more often, and drain life from them when you do, making you much more resilient indensely-packed dungeon brawls.
The best part of Andariel’s Visage is its unique aspect, however, which grants you up to a 20 percent chance to summon a poison nova on hit, dealing huge damage over time to all enemies within range. The damage here is high, and can be taken even higher when combined with class-specific skills. And while it only triggers 20 percent of the time at best, the increased attack rate this helm provides will ensure that you’ll still see poison novas fairly regularly. Unlike the helm itself, which you’ll see once per game if you’re lucky.
3Melted Heart Of Selig
All of the ‘ultra-rare uniques’ covered in this list break at least one of the core rules of Diablo 4, but the violations made by the Melted Heart of Selig feel particularly egregious. It provides a 30 percent boost to your maximum resource, be it energy, spirit, or essence, and allows your resource pool to act asa kind of barrier, absorbing damage for you while you have some available.

This completely changes how you approach building out your skills and gear, letting you use items that increase your maximum resource and resource regeneration rates as extra health boosts rather than for their intended purpose. On the other hand, it can also lead to scenarios where you can’t use your key skills during a heated battle, due to your resource being depleted by continuous enemy attacks. It’s an interesting amulet that changes how you play the game in a big way, making its ultra-rare status feel more than earned.
2Doombringer
With a name like Doombringer, you might expect this sword to create hugewaves of destructive darknesswith every swing, laying waste to all in their path. And you’d be right: up to 25 percent of the time, at least. This blade’s unique aspect is a lucky hit ability, with up to a one in four chance of creating an AOE shadow damage blast, hitting all nearby enemies and reducing the damage they deal by 20 percent for five seconds.
The fact that Doombringer is a fast weapon makes this more likely than you’d think given the raw odds, and also plays nicely with the lucky hit healing ability that also exists on the sword. Its other abilities increase your damage and maximum life, which are universally useful in all scenarios, making Doombringer one of the easiest ultra-rare uniques to slot into a given build.
1Harlequin Crest
This is the big one. While the other ultra-rare uniques are very powerful, and can even be build-defining in some cases, the Harlequin’s Crest is almost certainly the best item in all of Diablo 4. This isn’t due to the maximum life, resource generation, cooldown reduction, or stat boosts it offers, but rather due to its unique aspect, which provides four additional ranks to all of your skills.
This is an absurd bonus, and one that provides access to all of your classes' skills and all of their passives: something that literally isn’t possible with the number of skill points you’re normally allocated in the game. It makes your character better in every way, and pushes your favourite skills — those you’ve put five points into anyway — to new heights by increasing their rank to nine. No matter what class or build you’re running, the Harlequin Crest is Diablo 4’s Holy Grail, and the item by far the most deserving of the ultra-rare status and drop rate.
